1.  Waywest Lighting & Camera Inc. must be named Loss Payee and Additionally Insured.

2.  Certificate must note that coverage is provided for loss or damage to rented equipment for a limit not less than the value of rented equipment ($200,000 lighting & grip package; $40,000 truck; $80,000 per camera package).

3.  General Liability coverage must be in force for a limit of not less than $1,000,000 per occurrence and $1,000,000 aggregate.

4.  Third party property damage - no less than $1,000,000.

5.  Automobile Liability coverage must note "Hired Autos", "Non-owned Autos". No less than $1,000,000.

6. Workers Compensation - no less than $1,000,000.

7.  These phrases: "Endeavor to", and "but failure to mail such notice shall impose no obligation or liability of any kind upon the company, it's agents or representatives", must be excluded from the cancellation clause at the bottom right hand corner of the certificate of insurance form.
